Job description

What You'll Do: An individual who, under professional supervision, provides personal care services to agency clients.
Organizational Relationship: Reports to the Administrator
Responsibilities: Obtaining a working knowledge of the provisions of the NV statutes which govern the licensing of agencies before providing personal care services to clients of the agency. A copy of those statutes (NAC 449) will be provided at orientation.
Providing services/care per the client's service plan Attend required orientation/training upon hire & ongoing per agency policy.
Provision of personal care services to agency clients in their homes, according to the client service plan, to assist those persons with activities of daily living, including, without limitation: Elimination of wastes from the body
Dressing and undressing Bathing Grooming
The preparation and eating of meals
Laundry Shopping Cleaning Transportation
Any other minor needs related to the maintenance of personal hygiene Perform tasks assigned by the Administrator/designee.
Document/report in writing, on agency forms, to the Administrator/designee on each client assignment. Promptly report any significant changes observed or reported in the client's condition to the Administrator/designee. Confer as required with the Administrator/designee regarding client's progress.
Follow Universal precautions/proper infection control when providing client service.
Maintain client confidentiality per HIPAA privacy and Agency policy guidelines.
Perform ONLY tasks specified for each individual client on the service plan.
Follow emergency procedures in the event of any medical or nonmedical situations.
Follow client rights at all times.
Document/Record each activity performed on each case on a daily basis.
Document/Report any incidents or client changes immediately to the Administrator/designee.
Submit Daily Activity Sheets and record accurately on a timely basis (weekly).
Follow instructions, is punctual and is at work as scheduled.
Follow all established, written agency policy and procedures.
Attend in-service training annually per agency policy/state statutes.
Maintain effective communication with client, family, Administrator/designee and other team members.
Other duties as assigned.
Requirements
Qualifications: Be at least 18 years of age Be responsible and mature and have the personal qualities which will enable the attendant to understand the problems of elderly persons and persons with disabilities. Understand the provisions of NV Statutes governing PCS and chapter 449 of NRS; Demonstrate the ability to read, write, speak and communicate effectively with clients of the agency; Demonstrate the ability to meet the needs of the clients of the agency; and Receive annually not less than 8 hours of training related to providing for the needs of the clients of the agency. Criminal & other checks required First Aid & CPR training within 6 months of hire Successfully complete on hire orientation & training
Functional Abilities: Is able to carry bundles upstairs weighing up to 10 pounds. Must be able to stoop and bend. Must be able to travel to prospective client's place of residence. Must be able to hear and effectively communicate in English.
