Blue Sky, Inc. is seeking a Part-Time Case Manager to join our team. This position offers 20–25 hours per week and provides an opportunity to support vulnerable individuals and their families by helping them access services, achieve personal goals, and build meaningful connections within their communities.

Responsibilities

As a Case Manager, you will:

  • Coordinate services and supports for individuals and families.
  • Develop person-centered support plans based on assessed needs, goals, and preferences.
  • Monitor and evaluate services to ensure needs are being met.
  • Assist individuals in accessing community resources and services.
  • Help identify and connect with qualified service providers.
  • Educate individuals and their legal representatives about available service options.
  • Support self-direction, independence, and community integration.

Qualifications

The ideal candidate will be self-motivated, organized, and skilled at problem-solving. Successful applicants will be able to manage priorities effectively, work independently, and collaborate as part of a team.

Required Qualifications:

  • Bachelor's degree from an accredited four-year college or university in Social Work, Psychology, Sociology, or another Human Services-related field.

Compensation

  • Wage: $20.67–$23.08 per hour
  • Schedule: Part-time, 20–25 hours per week
