We are currently seeking a PHP (Partial Hospitalization Program) Therapist/Liaison for Riveredge Hospital. Apply online today.
The PHP (Partial Hospitalization Program) Therapist/Liaison provides intensive, structured mental health treatment to individuals who require more support than traditional outpatient care but do not need inpatient hospitalization. The therapist delivers individual, group, and family therapy while coordinating care with a multidisciplinary team.
Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans
  • Provide individual, group, and family therapy using evidence-based practices (CBT, DBT, trauma-informed care, etc.)
  • Complete pre-certs for insurance for new starts for PHP and IOP admissions to the program
  • Connect daily with the social services team to discuss step downs, and support with getting step downs admitted to the PHP, or IOP program
  • Support with setting up transportation services through insurance companies to support patients ability to attend the program
  • Facilitate psychoeducational and process groups in a structured PHP setting
  • Monitor client progress and adjust treatment plans as needed
  • Maintain accurate and timely clinical documentation (progress notes, treatment plans, discharge summaries)
  • Collaborate with psychiatrists, nurses, case managers, and other team members
  • Assess risk (suicidality, self-harm, harm to others) and implement safety planning
  • Provide crisis intervention when necessary
  • Coordinate discharge planning and aftercare referrals

Qualifications
