POSITION SUMMARY

Provide individualized case management, intervention support, and resource coordination to assist students in overcoming academic, personal, financial, and other barriers impacting persistence and credential completion. Assess student needs and connect students with appropriate internal and external resources using relationship-based coaching and problem-solving approaches. Support student retention initiatives through ongoing case management, trend identification, collaboration with campus partners, and recommendations related to student success strategies and interventions.

ESSENTIAL FUNCTIONS

• Assess student needs, barriers, and support concerns to determine and coordinate appropriate internal and external resources, interventions, and referral strategies.
• Using relationship-building coaching approaches, provide direct case management, advocacy, intervention planning, referral coordination, and comprehensive follow-up to assigned student cohorts. Assess complex student barriers and collaborate with students to identify individualized strategies, resources, and solutions that support persistence and educational goal completion. Advise students as they identify obstacles and barriers that may interfere with their educational goals. Help students explore options for overcoming these obstacles or barriers.
• Provide students with information about appropriate community service agencies, coaching students to successful access of resources needed to complete their credential.
• Respond to student referrals and outreach opportunities by assessing student needs, determining appropriate intervention approaches, and coordinating follow-up support and resource connections
• Contribute to identify trends and barriers impacting student persistence and credential and contribute recommendations and intervention strategies intended to support student success outcomes.
• Maintain accurate and up-to-date information about districtwide community service agencies, supports available to students, and the processes for accessing such resources.
• Maintain knowledge of internal resources (academic advising, career advising, academic coaching and tutoring, faculty mentoring, etc.).
• When needed, review programmatic criteria to individually assess applicants to determine eligibility for appropriate program.
• Contribute to the development of retention planning by considering relevant data and providing others at the College with trends in barriers to students.
• Collaboratively provide awareness of resources available to students throughout the NWTC district.
• Identify process gaps, student support barriers, and emerging student needs and collaborate with campus partners and leadership to recommend intervention strategies and process improvements.
• Serve as the primary point of contact for select community service agencies and work collaboratively with other NWTC staff and leaders to engage with all community service agencies.

MINIMUM QUALIFICATIONS AND WORK EXPERIENCE

• Associate degree
• 2-3 years social services experience or an equivalent combination of education and experience required.
• Experience or demonstrated commitment to engaging in service to underrepresented populations.

Preferred Qualifications:

• Bachelor degree in social work, human services, psychology, or other related field
• Bilingual
