Where You Will Work:
Older patients are at risk for loss of self-care abilities during the course of an acute medical illness that results in hospitalization. The Acute Care for Elders (ACE) Unit is a continuous quality improvement model of care designed to prevent the patient's loss of independence from admission to discharge in the performance of activities of daily living (hospital-associated disability).

The ACE unit intervention includes principles of a prepared environment that encourages safe patient self-care, a set of clinical guidelines for bedside care by nurses and other health professionals to prevent patient disability and restore self-care lost by the acute illness, and planning for transitions of care and medical care. By applying a structured process, an interdisciplinary team completes a geriatric assessment, follows clinical guidelines, and initiates plans for care transitions in concert with the patient and family.

The ACE Unit at Advocate Aurora Sinai is a great unit for nurses looking to work with elder population, new graduate nurses, and nurses interested in building a strong base for community and population health services.

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
  • Strong interest and passion for cardiac care

What You'll Do:

  • As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
  • Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.
