Job description

We are seeking a Part-Time (weekend) Rehabilitation Counselor to work in our Behavioral Health Services Division. This position provides care and supervision of all assigned clients to ensure a safe, healthy, secure, and pleasant environment. The selected candidate would help facilitate client growth and development towards independence in self-care, daily living skills, socialization, and community living, while participating in the development of the individual service plans and related documentation.

Some other responsibilities include the following:

  • Providing care and supervision of all assigned clients to ensure a safe, healthy, secure, and pleasant environment, which promotes growth and development.
  • Facilitating client growth and development toward independence in self-care, daily living skills, socialization,n and community living.
  • Participating in the development of the individual service plans and related documentation.
  • Reviewing and modifying individual service plans to accommodate growth and development.
  • Tracking all client progress, as identified in their individual service plans. Provide supplemental reports on individual growth and development, as required. Record their progress as they move through the service delivery system.
  • Serving as an advocate and encouraging clients to assume a proactive role in their rehabilitative process.
  • Providing guidance and support to all clients; teaching and reinforcing skill development in the areas of nutrition, basic self-care, routine medical care, and follow-up, as developed through the joint planning sessions of the individual service plans.
  • Ensuring the overall safety of assigned individuals by maintaining a hazard-free environment and avoiding potentially dangerous situations.

Qualifications:

  • High school diploma or equivalent required; Associate’s degree preferred in the social sciences or health care-related field and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
  • Good communication skills, both oral and written.
  • Ability to work independently and as part of a team.

Scope:

  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents.
  • Possess manual dexterity and fine motor skills.
  • May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs).
  • May require crisis intervention in emergencies.
  • May be required to report to work during emergencies, including inclement weather.
