As a Tarrant/Denton Pediatric RN Case Manager with The Visiting Nurse Association of Texas (VNA), you will work during normal business hours, Monday - Friday, 8:00 am - 5:00 pm.

Job Summary

In this role, you aren't just managing a caseload; you are the vital link providing clinical expertise, advocacy, and emotional stability for pediatric patients of all ages. 

Key Responsibilities

Full-Time Tarrant/Denton Pediatric RN Case Manager with primary functions to administer nursing care for pediatric patients in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care. 

  • Comprehensive Assessment: Identify physical, psychosocial, and environmental needs of pediatric hospice patients.
  • Care Planning: Implement, develop, and document individualized plans of care to ensure continuity and quality.
  • Clinical Leadership: Provide clinical direction to Hospice Aides and LVNs and assist in training new staff.
  • Patient Advocacy: Communicate effectively with families, team members, and healthcare professionals to meet patient needs.
  • Emergency Response: Perform admissions as needed and participate in on-call responsibilities for emergencies.

Qualifications

  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current Texas State RN License and a valid Texas Driver’s License.
  • Experience: At least one year of experience as a Pediatric RN in a clinical care setting is required. Hospice experience is preferred.
  • Skills: Strong interpersonal skills and the ability to read and write English.
  • Transportation: Reliable transportation with valid auto liability insurance for local travel to patient homes. 
  • Certifications: Proof of current CPR, a TB test, and a Hepatitis consent/declination

Environmental and Working Conditions: Works in patients home in various conditions; possible exposure to blood and bodily fluids and infections diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather. 

Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.
