Pay Rate: $30.00-$90.00 per hour

Job Description

Community Wellness Outpatient Mental Health Center, LLC is seeking an experienced Family Nurse Practitioner (FNP) to provide primary and integrated healthcare services across affiliated outpatient behavioral health, substance use treatment, psychiatric rehabilitation, supportive housing, and primary care programs.

This is a part-time W-2 role based in Baltimore, MD. The ideal candidate is a licensed Family Nurse Practitioner with experience in primary care, behavioral health, integrated care, addiction medicine, MAT services, clinical documentation, and interdisciplinary care coordination.


Key Responsibilities

  • The Family Nurse Practitioner will conduct medical evaluations, wellness visits, follow-up care, and clinical assessments for individuals enrolled in outpatient behavioral health, substance use treatment, psychiatric rehabilitation, supportive housing, and primary care programs.
  • This role will diagnose and manage acute and chronic medical conditions, prescribe medications within scope, and provide Medication-Assisted Treatment services, including induction, stabilization, and monitoring when appropriate.
  • The FNP will order, review, and interpret diagnostic tests, labs, screenings, medical histories, and treatment plans. This position will also develop individualized care plans, coordinate referrals to specialists, and educate clients and families on wellness, prevention, relapse prevention, medication safety, and treatment expectations.
  • The role will document services in ICANotes in accordance with COMAR, Medicaid, payer, and organizational standards. The FNP will also participate in interdisciplinary team meetings, case reviews, staff trainings, quality assurance activities, audit readiness, licensing inspections, and clinical compliance initiatives.
  • This position may provide consultation for complex cases and respond to after-hours emergencies as assigned.


Qualifications

  • Candidates must have active, unrestricted Maryland Board of Nursing licensure as a Certified Registered Nurse Practitioner (CRNP). A Master's degree in Nursing from an accredited institution and board certification as a Family Nurse Practitioner are required.
  • A valid DEA registration and Maryland CDS registration are required. Current CPR and First Aid certification are also required. Candidates must be able to obtain and maintain professional liability or malpractice insurance.
  • A minimum of 2-3 years of clinical experience in primary care, behavioral health, or integrated healthcare settings is preferred. Experience with Medication-Assisted Treatment services is strongly preferred. Experience working within interdisciplinary or integrated care environments is also preferred.


Skills

  • The ideal candidate will have strong clinical skills in primary care, integrated care, behavioral health, and addiction medicine. This role requires knowledge of COMAR, OHCQ, CARF, SAMHSA, DEA requirements, EHR documentation, ICANotes, and CPT coding.
  • The candidate should have strong communication, teamwork, clinical judgment, conflict-resolution, crisis response, and decision-making skills. A commitment to trauma-informed, person-centered care is required.
