JOB DESCRIPTION

Love your job and feel full filled as an In-Home Care Licensed Vocational Nurse. Family Tree Private Care is searching for full-time, part-time and PRN LVNs. We are looking for compassionate nurses with a passion for service, family values, and a true desire to help the elderly. Nursing positions at Family Tree Private Care will enjoy flexible schedule options, an enjoyable work environment that is warm, positive, and supportive.


RESPONSIBILITES

  • Travel to assigned patient’s homes and manage their care plan according to physician’s instructions.
  • Assess and chart observations of the patient’s condition at each visit.
  • Complete evaluation tasks, including reviewing medication and vital signs.
  • Dress or redress wounds and assess the healing progress.
  • Educating caregivers and family on the aftercare or ongoing care of the patient.
  • Keep the patient comfortable through palliative care.
  • Recommend tools or devices that may improve the quality of life for the patient.

SKILLS

  • Strong interpersonal skills that allow them to get to know their patients, build personal rapport and establish trust to provide high-quality care while increasing patients’ enjoyment and comfort.
  • Initiative so they can make independent decisions when required and accept direction and requests from patients.
  • The ability to follow instructions but also act independently if necessary.
  • Excellent observational and problem-solving skills
  • Ability to act swiftly in emergencies and work efficiently.

REQUIREMENTS AND QUALIFICATIONS

  • Relevant license and certification
  • A valid driver’s license and reliable transport
  • Strong candidates have a minimum of 1 year of experience working in the home care industry (or related industry – hospice, home health, hospital, etc.)
