Title: Family-Based Provider Liaison
Reports To: Executive Director
Classification: Part-Time
FLSA Status: Non-Exempt
JOB SUMMARY
The Family-Based Provider Liaison is responsible for recruiting and training new family-based providers for adults and children. Host and attend events and create marketing material to recruit new families. Be certified to train family-based providers in all areas and to complete all required paperwork.
ESSENTIAL JOB FUNCTIONS
• Supervise staff and schedule them for their assigned training and paperwork.
• Relay any concerns or problems with Family Base Providers to the Executive Director.
• Function as a liaison for family-based providers, DCS, and the Program Coordinator.
• Interview prospective individuals served and families.
• Coordinate and schedule appointments, meetings, and related activities.
• Attend all required meetings (staff, COS, PCSP, and training).
• Serve as a support for the individual served, their family, and the Evergreen staff who provide services.
• Ensure that all policies, procedures, regulations, and guidelines are followed.
• Complete home study in compliance with timelines.
• Work flexible hours as requested by the Program Coordinator or Executive Director.
• Evaluate Family-based Providers as needed.
• Participate actively in contracting with family-based providers.
• Conduct unannounced visits to facility sites as directed by the Program Coordinator or the Executive Director.

• Prioritizing and Organizing Work: Allocates time and attention based on what is most important to achieve key goals and objectives. Effectively organizes and balances tasks and priorities to keep multiple projects on track.
QUALIFICATIONS/EXPERIENCE/JOB KNOWLEDGE
• Preferred experience in serving individuals with disabilities or four (4) years full-time equivalent experience in a supervisory position, preferably in the field of mental health or a related field.
• Preferred four (4) year degree in human services, social work, or related field.
• Experience in supervising staff and following licensing and regulatory guidelines.
• Needs a clear understanding of administrative procedures and personnel management.
• Needs an understanding of Medicaid guidelines as they relate to programming.
• Working knowledge of Google Workspace and the ability to learn other software programs as required for this position.
PHYSICAL REQUIREMENTS
• Constantly moves about to coordinate work.
• Regularly works in a fast-paced environment with multiple task deadlines.
• Regularly moves and positions objects weighing up to 50 pounds.
• Occasionally exposed to viruses and infectious conditions.
• Constantly alert and observant during working hours.

SUPERVISORY RESPONSIBILITIES
• Will supervise assigned staff/families.
SPECIAL REQUIREMENTS
• Must have understanding, patience, and tact in dealing with individuals served and their families.
• Must be creative in problem-solving and planning.
• Must have the ability to maintain good working relationships with staff, professionals, and persons from other agencies.
SKILLS AND ABILITIES
• Working knowledge of Google Workspace.
• Aptitude to learn other software programs as required for this position
EMPLOYMENT VARIABLES
• Must have a good driving record that meets the minimum requirements for Evergreen and reliable transportation.
• Must pass drug screen and criminal background check.
• May be asked to work some evenings when required.
• Extensive travel in your company or personal vehicle (mileage role will apply).

• May be required to work in a variety of settings and environments, both indoors and outdoors.
