POSITION SUMMARY

Provide direct care, patient education and the management of the applicable therapy (physical, occupational or speech) called for in the patient’s treatment plan. Consult with and educates other care team members concerning applicable patient assessment and treatment protocols. Serve as a rehabilitation resource for the care team.

JOB SPECIFIC EXPECTATIONS

Patient Care Skills:

  • Provides those services requiring substantial and specialized therapy skills, as required per skills competency check list.
  • ST assess patients for admission to the home health program. Admits patients for care based on skilled need and payer source eligibility requirements.
  • Initiates preventive and rehabilitative therapy procedures as appropriate for the patient s care and safety.
  • Develops and revises plan of care and discharge plan in conjunction with patient, family, physician, and care team to assist patient in reaching or maintaining a maximum level of independence.
  • Evaluates the patient’s response to therapy services, assesses service needs on an ongoing basis, arranges additional therapy services required, and obtains physician s orders for such services.
  • Educates the patient, family, and care team members regarding disease process, self-care techniques and prevention strategies.

Documentation:

  • Documentation demonstrates compliance with Medicare, Medicaid, and other regulatory requirements.
  • Documentation reflects notification of physician when patient’s status changes and revision to the plan of care as appropriate.
  • Documentation of visits reflects the patient’s response to care and assessment of psycho-social needs through out the course of care.
  • Clinical documentation and Oasis time points are complete and accurate and submitted to office per policy and guidelines with an error rate of less than 5%.
  • Documentation reflects complete assessment/reassessment of patient and includes goals, plan, and evidence of discharge planning.

Management:

  • Supervises the care delegated to the PTA/COTA and/or HHA (activities appropriate for the HHA, PTA or COTA to perform.)
  • Provides supervisory visits per company policy and conditions of participation to HHA and/or PTA/COTA.

Regulatory:

  • Clinical record documentation demonstrates compliance with physician orders for care provided and frequency of visits made.
  • Obtains physician s orders for care and services provided per policy.
  • Complies with company s policies and procedures.

MINIMUM JOB REQUIREMENTS:

  • Graduate of an accredited School of Physical Therapy/ Occupational Therapy/Speech Pathology with a Baccalaureate or Master s Degree.
  • Must have a current licensure by the Georgia State Board of the specified therapy and a valid CPR certification.
  • Must possess a current valid Driver’s license.

WORK SCHEDULE:

  • 20 Hour work week
