Key Responsibilities & Clinical Duties

  • Conduct 6 to 7 scheduled in-home member diagnostic visits per 8-hour shift within a designated geographic territory.
  • Manage travel efficiency utilizing an optimized route structure averaging 20 minutes of drive time between patient homes.
  • Perform baseline Biometric Screenings at every visit, including Blood Pressure, Weight, and Oximetry.
  • Handle, label, and preserve all laboratory specimens according to strict clinical protocols for transport and processing.
  • Document clinical findings and data accurately in real-time using Porter's mobile electronic health platform to trigger appropriate follow-up care from our virtual Care Guides.
  • Execute customized, on-site laboratory specimen collections and testing based on individual member care gaps, including:
    • FIT Testing (COL) for colorectal cancer screenings.
    • HbA1C (HBD) for diabetes monitoring.Urine Microalbumin (KED) for early kidney health screening.
    • Glomerular Filtration Rate (GFR) blood collection.
    • Prostate Specific Antigen (PSA) blood screening.

Coverage Requirements: Must be willing to see patients across the following counties. Note: You will likely be in only 1-2 counties per shift: Chatham and Effingham

  • Strong phlebotomy skills and comfort handling portable laboratory/diagnostic equipment.
  • Ability to work independently with excellent time-management skills to maintain a 6 to 7 visit daily cadence.
  • Valid driver’s license, reliable personal vehicle, and clean driving record required for daily field travel.
