Job description

At Curana Health, we’re on a mission to radically improve the health, happiness, and dignity of older adults—and we’re looking for passionate people to help us do it.

As a national leader in value-based care, we offer senior living communities and skilled nursing facilities a wide range of solutions (including on-site primary care services, Accountable Care Organizations, and Medicare Advantage Special Needs Plans) proven to enhance health outcomes, streamline operations, and create new financial opportunities.

Founded in 2021, we’ve grown quickly—now serving 200,000+ seniors in 1,500+ communities across 32 states. Our team includes more than 1,000 clinicians alongside care coordinators, analysts, operators, and professionals from all backgrounds, all working together to deliver high-quality, proactive solutions for senior living operators and those they care for.

At Curana Health, we’re reimagining how seniors receive compassionate, comprehensive psychiatric care. We are seeking an experienced and mission-driven Psychiatric Nurse Practitioner (PMHNP) to join our growing team and provide high-quality mental health support across Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living (AL), and Independent Living (IL) communities.

This is more than a job—it’s an opportunity to make a lasting difference in the lives of adults and older adults while enjoying a supportive environment and true work-life balance.

As a Psychiatric Nurse Practitioner at Curana Health, you’ll help shape the future of geriatric behavioral health care. You’ll work alongside a collaborative, multidisciplinary care team to deliver personalized psychiatric services, ensuring continuity of care across the post-acute and long-term care continuum. Your expertise will empower residents, support families, and improve outcomes for an underserved population.

  • Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
  • Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
  • Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
  • Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
  • Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary.
  • Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices.
  • Acute Care Collaboration: Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
