What You'll Do:
The Registered Nurse is responsible for providing clinical nursing oversight and support to consumers residing in residential behavioral health programs. This role involves conducting nursing assessments, monitoring health status, coordinating care with healthcare providers as needed, providing health education, reviewing clinical documentation, and offering nursing recommendations to ensure consumers receive safe, effective, and person-centered care.
Key responsibilities include:
  • Conduct comprehensive nursing assessments upon admission and as clinically indicated to identify physical, behavioral health, and wellness needs of consumers.
  • Monitor and evaluate consumers' health status, documenting findings and reporting significant changes to appropriate team members and healthcare providers.
  • Review and monitor assigned consumer charts to ensure accuracy, completeness, regulatory compliance, and adherence to agency policies and procedures.
  • Provide clinical support, guidance, and nursing recommendations to residential staff regarding consumer health needs, medication management, and treatment planning.
  • Collaborate with psychiatrists, primary care physicians, nurse practitioners, pharmacists, and other healthcare professionals to coordinate and optimize consumer care.
  • Communicate effectively with healthcare providers regarding medical concerns, treatment recommendations, medication changes, laboratory results, and follow-up care.
  • Provide health education and wellness coaching to consumers, families, and staff on topics including medication management, chronic disease prevention, behavioral health conditions, nutrition, and healthy lifestyle practices.
  • Assist with care coordination activities, including scheduling and follow-up for medical appointments, specialty consultations, and diagnostic testing.
  • Monitor consumer compliance with prescribed medications and treatment plans and address barriers to adherence.
  • Participate in interdisciplinary team meetings and contribute nursing assessments and recommendations to support individualized service plans.
  • Review laboratory results, medical reports, hospital discharge summaries, and other clinical documentation to ensure appropriate follow-up care.
  • Respond to clinical concerns and provide crisis intervention support within the scope of nursing practice.
  • Ensure compliance with applicable state, county, agency, and regulatory requirements governing behavioral health and residential services.
  • Maintain accurate, timely, and confidential clinical documentation in accordance with organizational standards and regulatory guidelines.
  • Participate in quality improvement activities, audits, and accreditation processes as required.
  • Serve as a clinical resource for residential program staff and promote best practices in consumer care.

Who You Are:
  • Education: Bachelor of Science in Nursing (BSN) preferred.
  • License/Certification: Current Registered Nurse (RN) license in the State of New Jersey (or applicable state).
  • Experience: Minimum of 2 years of nursing experience; experience in behavioral health, psychiatric nursing, community health, residential services, or case management preferred.
    • Knowledge of behavioral health diagnoses, psychotropic medications, chronic disease management, and healthcare regulations.
  • Requirements: Proficiency with electronic health records and clinical documentation systems.
  • Skills: Strong assessment, clinical judgment, and care coordination skills. Excellent written, verbal, and interpersonal communication skills. Ability to work collaboratively with multidisciplinary teams and external healthcare providers.
  • This role requires driving as an essential function. Candidates must have a valid driver's license and be subject to post-offer verification of driving eligibility consistent with company insurance requirements.
