Job description

Athena Hospice of Rhode Island is looking for a part-time RN Case Manager!

This is an on-the-road position with mileage reimbursement. 8a-4:30p schedule. We service the entire state of Rhode Island. $45 to $50 an hour, depending on experience, with weekly pay.

You plan, organize, and direct the hospice care of assigned patients under the direction of the clinical director.

Experience & Education:

  • Must possess an active and unencumbered RN license in Rhode Island.

  • Must have a nursing degree from an accredited college, university, or school.

  • Prior experience in hospice is required.

Duties & Responsibilities:

  • Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs.

  • Provides a complete physical assessment and history of current and previous illnesses.

  • Supervises hospice aides.

  • Provides professional nursing care by utilizing all elements of the nursing process.

  • Assesses and evaluates the patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care.

  • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.

  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

  • Counsels the patient and family in meeting nursing and related needs.
