Job Description

Promoting and maintaining continuity of nursing services for the residents. Responsible for planning, implementing, and evaluating, all nursing services during assigned shifts. Supervising, interpreting performance, counseling, and directing CNAs. Acts as a role model and effective team member for stimulating growth and harmony between co-workers and peers. Personally committed to enhance the lives of each resident by providing the highest quality of life possible.

EDUCATION, EXPERIENCE, CERTIFICATION AND LICENSURE REQUIREMENTS

  1. Graduate of an accredited school of nursing with a diploma or degree.
  2. Licensed as a Registered Nurse in Virginia.
  3. CPR/AED certification.
  4. 2+ years of nursing experience.

Job Duties

  1. Responsible for the total nursing services of assigned residents.
  2. Receives report from off-going shift and gives report to oncoming shift and make rounds.
  3. Receives physician orders and carries out orders according to established policies and procedures.
  4. Makes shift rounds with physicians.
  5. Knowledgeable of residents’ conditions. Reports significant change to their assigned supervisor and/or Physician.
  6. Responds to emergency calls of residents as assigned.
  7. Administers medications in accordance with regulations and established policy and procedures.
  8. Administers treatments according to physicians’ orders. Administers subcutaneous fluids. RNs to administer intravenous fluids unless LPN has had specialized training. Regularly checks on residents receiving IV fluids.
  9. Implements established resident care policies and procedures.
  10. Orders and stores medications according to the established pharmaceutical policies and procedures. Contacts their assigned supervisor or Director of Nursing, and/or Physician if there are any questions regarding resident’s medication or treatment, implements established stop-order policy, and removes all outdated and discontinued drug medications and places them in designated area.
  11. Maintains current scheduled and controlled drug record in accordance with State and Federal laws.
  12. Maintains and revises nursing care plan and service plans of residents as assigned.
  13. Maintains residents’ clinical records in accordance with established policies and procedures.
  14. Supervises and evaluates assigned certified nursing assistants and medication aids, including resident assignments and other assignments such as cleaning of baths, utility rooms, and linen rooms.
  15. Sees that all equipment is working properly and that a safe environment is maintained in assigned areas. Promptly completes maintenance request forms.
  16. Records required information in the electronic medical records per established procedures. Records monthly summaries on residents’ charts as assigned. Record vitals as assigned and keep the care team informed of diet changes.
  17. Assesses resident injuries and renders first aid or makes decisions for appropriate referral to doctor and/or ER. Completes incident report.
  18. Maintains a physical, social, and psychological environment conducive to the wellbeing of the resident using the neighborhood/ household model. Encouraging socialization, and rehabilitation of residents.
  19. Attends mandatory in-service education and completes Relias/Learning in-services assigned on the computer.
  20. Regularly attends nursing team members meetings and keeps their assigned supervisor and Director of Nursing informed of concerns and grievances.
  21. Assists nursing assistants with residents’ care, as necessary including bathing and eating assistance.
  22. All other duties as assigned.

Knowledge, Skills, and Abilities

  1. Intermediate conceptual knowledge of theories, practices, and procedures within nursing, typically acquired through a nursing degree.
  2. Intermediate verbal and written communication skills required.
  3. Demonstrates working knowledge of common disease processes and treatments.
  4. Clinical skills sufficient to perform all essential job duties.
  5. Ability to read, write and speak using the English language.
  6. Ability to work in a fast-paced environment with multiple tasks
  7. Ability to see and hear and react appropriately to alarms and notifications.
  8. Computer knowledge and skills with an aptitude to learn company software

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

  1. May include frequent lifting (up to 50lbs).
  2. Capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching, kneeling, reaching over the head, grasping with both hands, and fine manipulation.
  3. Physically able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment.
  4. Ability to work inside and outside to effectively make rounds daily throughout the facility and to respond to problems and emergencies.
  5. Normal eyesight or use corrective lenses to achieve it.
  6. Ability to hear and distinguish between normal tones and be able to perceive the nature of sounds.
  7. Universal staffing, holiday, inclement weather, and a flexible work schedule are required to meet the needs of the job.
  8. Exposure to the sights, sounds and odors of a health services environment including senior citizens with serious cognitive loss.

SUPERVISION and Decision Making EXERCISED

  1. Moderate level of guidance and direction – work is reviewed at critical points for soundness of judgement and overall adequacy.
  2. Independent decisions are made based on standard policies and practices.
  3. Ability to assess, develop, and carry out resident plans and guide team members.
  4. Problem solving is more complex. Problems deal with individual and specialized situations. May have to collaborate with senior level nurses on some occasions.
  5. Ability to recognize and adequately handle difficult problems and scenarios with residents and help supervise team members accordingly.
