Talem Home Care is seeking a detail-oriented and knowledgeable part-time Registered Nurse or Nurse Practitioner to be responsible for conducting in-home evaluations for clients enrolled in Colorado’s Medicaid In-Home Support Services (IHSS) program, as well as clients with other payers.

Job Summary
The Registered Nurse (RN) or Nurse Practitioner (NP) is responsible for conducting comprehensive in-home evaluations for clients enrolled in Colorado’s Medicaid In-Home Support Services (IHSS) program, as well as clients covered by other payer sources. This role ensures high-quality, compliant care through direct home visits, collaboration with clients and caregivers, and ongoing assessment of care needs. The RN/NP reviews and updates care plans, validates caregiver competencies, provides training as needed, and completes all required clinical documentation. The position also includes post-discharge evaluations and caregiver skill validation for non-Medicaid clients, supporting safe, effective care delivery in the home setting.
 
Responsibilities 
  • Performing home visits during caregiver shifts.
  • Reviewing care plans with the client and caregiver.
  • Identifying and documenting any required changes in the care plan.
  • Provide a client intake assessment 
  • Certifying caregiver proficiency in care tasks.
  • Completing all required documentation related to these evaluations.
  • Providing post-discharge evaluations
  • Provide in-person trainings for the caregivers, if needed. 
Qualifications
  • Active, unrestricted Registered Nurse (RN) or Nurse Practitioner (NP) license in the State of Colorado
  • Graduation from an accredited nursing program (ADN, BSN, MSN, or DNP as applicable)
  • Minimum of 1–2 years of clinical nursing experience, preferably in home health, long‑term care, Medicaid services, or community-based care
  • Knowledge of care plan development, evaluation, and modification
  • Ability to assess caregiver competency and certify proficiency in assigned care tasks
  • Experience completing clinical documentation accurately and in compliance with state and payer requirements
  • Strong clinical assessment, observation, and critical‑thinking skills
  • Excellent communication and interpersonal skills with clients, caregivers, and interdisciplinary teams
  • Ability to work independently in home settings with minimal supervision
  • Valid driver’s license, reliable transportation, and ability to travel to client homes
