Home Health Registered Nurse (RN) *SIGN ON BONUS up to $10,000*

PHC Home Health, a division of Personal Home Care of North Carolina is currently hiring for a PRN/Part Time/Full Time Registered Nurses (RN) for the following areas: Waxhaw/Monroe/Indian Trail/Charlotte
Home Health Registered Nurses are responsible for performing general nursing duties for patients under a physician's direction at the patient’s home or assisted living facility.
Home Health RN Job Functions and Schedule will vary, but will generally include:
  • Provision of nursing care; including assessments, treatments, diagnostic procedures, and patient education based on the plan of care and physician’s orders.
  • Observe signs and symptoms, reactions to treatment, drug response/interactions, and changes in the patient’s condition; with appropriate follow-up in care and reporting to supervisor or physician as needed
  • Coordinates care, including care management and discharge planning as needed with the patient/responsible party, physician, other disciplines, and/or internal and external resources.
  • Documents accurately, objectively, and timely by regulations and agency policy/procedures.
  • Participates in multi-disciplinary team meetings weekly.
  • Participates in learning and job-enhancement activities as directed by the supervisor.
  • Participates in On-Call rotation. One weekday on-call is required.
Home Health RN Job/Qualification Requirements
  • Prior Home Health Experience is preferred
  • OASIS Knowledge is a plus
  • Excellent time management, organizational, and reporting skills
  • Strong communication skills, both written and verbal
  • Great technology skills
  • Current NC or multi-State RN License
  • Current Driver’s License and Automobile liability insurance issued in NC
