Purpose of Position

The Registered Nurse (RN) delivers skilled nursing services and treatments requiring specialized clinical expertise. Care is provided in accordance with state and federal regulations, professional nursing standards, payor requirements, and Interim HealthCare policies and procedures.

 
Essential Duties
  • Participate in interdisciplinary patient assessments to evaluate needs and support comprehensive care planning.

  • Collaborate with patients, caregivers, and representatives to develop and evaluate individualized care plans.

  • Incorporate patient goals and preferences into the care plan to ensure personalized, patientcentered care.

  • Provide physicianordered services and ensure all clinical directives are followed accurately.

  • Educate and counsel patients and families to promote understanding, selfcare, and informed decisionmaking.

  • Initiate preventative and rehabilitative nursing interventions to support optimal health outcomes.

  • Complete timely and accurate clinical documentation, updating care plans as needed.

  • Coordinate referrals to additional services to ensure continuity and comprehensiveness of care.

  • Monitor patient progress, evaluate outcomes, and adjust interventions as necessary.

  • Communicate effectively with the healthcare team to support seamless care coordination and discharge planning.

 
Required Qualifications
  • Graduate of an accredited nursing program.

  • Current Registered Nurse (RN) license in the state of practice.

  • Valid CPR certification.

  • Valid Driver’s License and auto insurance.

  • Must pass a drug screening.

 
Preferred Qualifications
  • At least 1,600 hours of RN experience or completion of the office’s preceptor program.

  • Experience in home health, IV therapy, or wound care.

 
Working Conditions
  • Frequent travel within the service area from patient to patient.

  • Meets all health requirements necessary for patient care.

  • Care is provided in patient homes or occupational settings.

  • Exposure to communicable diseases, bloodborne pathogens, and hazardous materials; adherence to safety protocols required.

  • Frequent standing, walking, bending, kneeling, and reaching.

  • Ability to lift, reposition, and transfer patients.

  • Medium physical activity, including occasional lifting/pushing/pulling up to 50 lbs.
