Job description

A California-licensed Pharmacist is sought for a remote Medication Therapy Management (MTM) pilot program. This part-time contract position offers flexible scheduling of 10-20 hours per week with a guaranteed minimum of 8 paid hours weekly, providing the ability to work remotely from anywhere within California. The role supports MTM services, patient counseling, and helps launch a clinical pharmacy initiative. Candidates must currently reside in California and be enrolled in Medicaid as an ORP provider.

Location: Remote (within California)

Schedule:

  • Approximately 10-20 hours per week, day shift hours for patient outreach and counseling
  • Flexible scheduling with a guaranteed minimum of 8 paid hours per week regardless of workload

Responsibilities:

  • Conduct Medication Therapy Management consultations
  • Provide patient counseling and medication education
  • Review medication profiles to identify opportunities for optimization
  • Document clinical interventions and patient encounters accurately
  • Collaborate with program stakeholders to support positive patient outcomes
  • Maintain compliance with all applicable state and federal regulations

Required Qualifications:

  • Active and unrestricted California Pharmacist license
  • Current residency in California
  • Previous experience with MTM required

  • Ability to work independently in a remote setting with flexible hours
