Skilled Nursing Facility amp; Post-Acute Care โ€“ Telemedicine amp; Behavioral Health Provider (NP/PA/PMHNP)Position Overview

We are building a collaborative team of Nurse Practitioners, Physician Assistants, and Psychiatric-Mental Health Nurse Practitioners passionate about delivering high-quality telemedicine care to skilled nursing and post-acute patients.

This role combines acute and routine medical care with behavioral health support, including a portion of encounters focused on common psychiatric and behavioral concerns.

This position is ideal for clinicians who enjoy clinical decision-making, autonomy, and meaningful collaboration with bedside nursing teams while helping support safe treat-in-place care and appropriate escalation when needed.

Providers care for a geriatric and post-acute population with varying degrees of medical, cognitive, and psychiatric complexity. Clinicians are supported by structured protocols, comprehensive onboarding, ongoing mentorship, and a team-oriented telemedicine environment.

Clinical Scope amp; Patient Population

Providers care for a diverse geriatric and post-acute skilled nursing facility (SNF) population. Responsibilities include:

  • Respond to urgent and acute calls from skilled nursing facilities.
  • Complete scheduled medical and behavioral health follow-up visits, including regulatory visits, medication monitoring, GDR-related follow-up, and other routine reevaluation needs.
  • Evaluate and manage common SNF medical concerns via telemedicine.
  • Evaluate common psychiatric, behavioral, and cognitive concerns within scope of practice.
  • Assess changes in mood, anxiety, sleep, cognition, behavior, and mental status.
  • Provide clinical recommendations, treatment plans, and escalation guidance.
  • Determine when emergency transfer or a higher level of medical or psychiatric care is appropriate.
  • Provide temporary medical and behavioral health coverage during planned or unexpected provider absences and other coverage gaps across Telos facilities.
  • Collaborate with nursing staff, facility providers, psychiatry/behavioral health clinicians, and telehealth leadership.
  • Document encounters accurately and efficiently within the EHR.
  • Follow established clinical protocols and workflows while utilizing independent clinical judgment.
  • Participate in onboarding, training, and occasional team meetings.

Providers must be comfortable independently evaluating acute changes in condition in the SNF setting using telemedicine workflows, bedside nursing collaboration, and established escalation protocols.

Required
  • Active, unencumbered Nurse Practitioner (APRN/APN), Physician Assistant (PA-C), or Psychiatric-Mental Health Nurse Practitioner (PMHNP) license.
  • Current DEA registration.
  • Active license and DEA registration in at least one of the following states, with willingness to quickly obtain additional licenses as needed:
    • Kentucky
    • Pennsylvania
    • Tennessee
    • Florida
    • North Carolina
  • Masterโ€™s degree or higher from an accredited Nurse Practitioner or Physician Assistant program.
  • Strong communication and clinical decision-making skills.
  • Ability to work effectively and independently in a fast-paced telemedicine environment.
  • Comfort evaluating and managing common mental health, behavioral, and cognitive concerns within scope of practice.
  • Willingness to participate in both medical and behavioral health/telepsychiatry-related encounters.
Preferred
  • At least 1 year of experience in a skilled nursing facility (SNF), long-term care, post-acute, hospital, emergency, or similar clinical setting.
  • Experience caring for geriatric populations.
  • Telehealth experience.
  • Experience managing common psychiatric conditions such as depression, anxiety, insomnia, behavioral disturbances, and cognitive disorders.
  • Formal behavioral health, psychiatry, or geriatric psychiatry experience is strongly preferred.
  • Dual certification in primary care and psychiatric-mental health is highly desirable.

Scheduling Philosophy

We believe flexibility and sustainability matter.

Our scheduling model is designed to support excellent patient care while maintaining a sustainable schedule for our providers. Scheduling is collaborative; however, providers must be flexible and willing to help meet evolving coverage needs and scheduling gaps as the telemedicine service line grows.

Scheduling Expectations
  • Scheduling flexibility: Must be willing to help cover varying shifts and scheduling needs based on program coverage.
  • Minimum commitment: 24 hours per week (two 12-hour shifts per week), Sundayโ€“Saturday.
  • Weekend requirement: One to two weekend shifts per month.
  • Day shift: 7:00 a.m.โ€“7:00 p.m.
  • Night shift: 7:00 p.m.โ€“7:00 a.m.
  • Night shift requirement: One to two night shifts per month.
  • Holiday coverage: Participation in one major or minor holiday annually, with advance scheduling and collaborative planning.
  • Additional shifts: Opportunities to pick up additional shifts as the service line grows.
