  • Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
  • Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents
  • Contributes toward quality assurance and performance improvement initiatives
  • Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience

Qualifications:

  • Current state licensure as Licensed Practical Nurse (LPN)
