How you'll make an impact in this role
  • Orchestrate the overall plan of care from admission to discharge, integrating patient and family preferences into a cohesive strategy across the entire continuum of care.
  • Serve as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment.
  • Pro-actively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity.
  • Maintain a continuous dialogue with the care transition team to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
