* Evaluates the patient & home health environment to determine required services
* Serves as the patient's advocate by providing information
* Responsible for timely completion & synchronization of OASIS visits as required & developing the POC based on findings from evaluation visit
* Involves patient & patient's family in developing the plan of care

Qualifications

We seek the following principal qualifications:
* RN state licensure in good standing
* Bachelor or Masters level degree in Psych & experience in mental health
* Medicare, home healthcare, and OASIS experience
