Homecare with Heart is looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $26 per hour, RN: $30 per hour depending on experience. The LPN provides skilled visits in patients' homes.
This position will be serving Trumbull County.
Responsibilities
What You'll Do:
• Administer medication in accordance with standards of practice.
• Encourage social\rehabilitative skills and encourage self-care and independence.
• Perform client treatments in accordance with Plan of Care
• Assist the consumer in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in-services, orientation and staff meetings.
• Provide service in accordance with agency's philosophy, standards, policies and scope of practice • Participates in Performance Improvement and adheres to HIPAA guidelines.
• Other duties as assigned.

What You'll Bring to the Team:
  • Valid State LPN/RN License
  • Valid driver's license and use of automobile is required due to the travel involved.
  • Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
  • Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
  • Must be able to use a computer, laptop or tablet.
  • Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
  • Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served.
  • Oasis experience required.
