Job description

PSC Community Services, Licensed Home Care Services Agency (LHCSA) is seeking for qualified Register Nurses (RNs) to conduct field visits to patient home and perform assessments in Brooklyn, Queens and Manhattan, as well as some administrative tasks for nursing department (if it's a part-time or full-time employment).
Job Requirements:
- Current NY RN license in good standing
- Strong assessment, critical thinking, and communication skills
- Reliable transportation and ability to travel to patient homes
- Weekday and weekend availability
Responsibility include:
- Perform initial assessments on new patients to determine eligibility and needed services
- Conduct ongoing assessments and monitor/evaluate patient status and progress
- Develop, update, and modify individualized plans of care
- Supervise home health aides
- Educate patients and families
- Communicate with physicians, other healthcare providers, and community resources
- Maintain accurate clinical documentation and records
- Comply with regulations, policies, and procedures
Job Type:
Per- diem (no benefits) $80 per visit, $100 per initial visit
part-time (benefits: sick time) $45-55 per hour, depending on your experience and qualifications
