Job description

Visually observes the patient at all times to prevent causing harm to self or others, or to prevent dislodging of tubes, drains, IV's, etc. Insures patient is not left unattended.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
• Assist patient requiring minimal standby assist to BR., chair, or ambulate in hall as directed by nurse.
• Relays basic patient information to staff and informs unit staff whenever additional assistance is required in the individual patient room.
• Participates in casual conversation and diversional activities with the patient such as playing cards, games, etc., if approved by the nurse.
• Provides comfort measures such as adjusting bed and pillows, and assist patients to reposition for comfort, when appropriate as directed by nurse.
• Assists patient/family with menu selection.
• Performs other duties within scope of practice consistent with purpose of job as directed by nurse.
• Obtains verbal report from the patient's nurse/patient care technician to include information regarding the patient's identified care needs, diagnosis, visual, auditory, or language barriers, as well as any special consideration (NPO, bed alarms, etc.).
• Positions self in patient room within arm's reach at bedside to maintain constant visual observation of the patient. Never leaves the patient alone or out of sight unless specifically instructed by the nurse.
• Accurately and legibly records data collected and notifies RN of any untoward events, i.e., behavioral changes, violent behavior, etc.
Preferred:
• High school diploma or GED.
• Completion of nursing assistant program.
• One year related experience.
