Mission Valley Post Acute
Position: MDS Coordinator RN
Employment Type: Part-Time
Pay Range: $50.00-$52.72 per hour
Join Our Clinical Leadership Team!
Mission Valley Post Acute is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our team on a part-time basis. The MDS Coordinator will be responsible for coordinating the facility's MDS process, ensuring accurate documentation, timely assessments, and compliance with applicable regulatory and reimbursement requirements.
This is an excellent opportunity for an experienced RN who is highly organized, knowledgeable in MDS processes, and passionate about maintaining high-quality resident care.
Key Responsibilities
  • Coordinate and oversee the facility's MDS assessment and care planning process
  • Ensure MDS assessments are completed accurately and within required timeframes
  • Collaborate with nursing, rehabilitation, dietary, social services, and other interdisciplinary team members
  • Review resident medical records and clinical documentation for accuracy and completeness
  • Participate in care plan meetings and ensure individualized care plans are developed and maintained
  • Monitor changes in resident condition and ensure appropriate documentation and assessments are completed
  • Support accurate documentation for reimbursement and regulatory compliance
  • Identify documentation discrepancies and work with appropriate departments to resolve them
  • Maintain compliance with federal, state, and facility MDS requirements
  • Assist with audits and survey preparation related to MDS and clinical documentation
  • Communicate effectively with residents, families, physicians, and the interdisciplinary care team

Qualifications
  • Current Registered Nurse (RN) license in the state of California
  • Previous MDS Coordinator experience in a skilled nursing or post-acute setting preferred
  • Strong understanding of the MDS 3.0 process and long-term care regulations
  • Knowledge of care planning, clinical documentation, and reimbursement requirements
  • Excellent organizational and time-management skills
  • Strong attention to detail and accuracy
  • Ability to work independently while collaborating effectively with an interdisciplinary team
