Position Summary

Reliable Home Care Providers, Inc. is seeking a highly skilled, detail-oriented, and experienced Quality Assurance Nurse (RN or LPN) to join our Home Health team on a part-time basis (20 hours per week). This position plays a vital role in ensuring the delivery of safe, compliant, and high-quality patient care in accordance with Medicare Conditions of Participation (CoPs), agency policies, and best clinical practices.

The Quality Assurance Nurse will work closely with clinical leadership and field staff to monitor documentation quality, ensure regulatory compliance, identify opportunities for improvement, and support the agency's continuous quality improvement initiatives.

Responsibilities

  • Conduct quality assurance audits of patient records, care plans, and clinical documentation.
  • Review and validate at least 6–8 OASIS assessments per day, depending on workload, for accuracy, completeness, and compliance (no coding required).
  • Review and audit at least 30 nursing notes per day, depending on workload.
  • Perform OASIS record reviews within agency-defined timeframes.
  • Identify documentation deficiencies, trends, and areas of risk, and recommend corrective actions.
  • Collaborate with nurses and interdisciplinary team members to ensure compliance with Medicare regulations, agency policies, and clinical standards.
  • Provide education, guidance, and feedback to clinicians regarding documentation quality and regulatory compliance.
  • Participate in quality improvement initiatives, interdisciplinary meetings, and performance improvement activities.
  • Maintain accurate, organized, and complete records of all quality assurance activities.
  • Demonstrate thorough knowledge of current Medicare Home Health regulations and Conditions of Participation (CoPs).
  • Support agency preparedness for surveys, audits, and accreditation reviews.


Requirements:
  • Active and unrestricted Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
  • Minimum of one year of home health experience required.
  • Strong experience with OASIS review and home health documentation compliance.
  • Thorough knowledge of Medicare Home Health regulations and Conditions of Participation (CoPs).
  • Proven ability to consistently review at least 6–8 OASIS assessments per day or at least 30 nursing notes per day, depending on workload.
  • Excellent organizational, analytical, and communication skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Exceptional attention to detail and commitment to delivering high-quality patient care.
