About the Opportunity
Join a growing mobile healthcare team delivering bedside musculoskeletal and pain management services across Skilled Nursing Facilities (SNFs) and post-acute care settings throughout the Upper Cumberland region.
This opportunity is ideal for semi-retired physicians, physicians seeking supplemental income, or providers already practicing in post-acute care who want a flexible, low-administrative role. We manage scheduling, billing, compliance, and operational support, allowing you to focus on patient care while collaborating with an experienced Physician Assistant team.
$3,000-$6,000/month (depending on how many PAs you support) for genuinely light, flexible collaboration on your own schedule: roughly an hour a week of chart review per PA, plus one on-site visit a month. No call, no weekends, no overhead. The stipend is for that light oversight - the real income comes when you treat your own patients at the bedside. You see the patients; we handle all the billing; you earn a competitive, uncapped share of the collections on the care you personally provide.

Key Responsibilities
Serve as the collaborating physician under a Tennessee Collaborative Practice Agreement (Tenn. Code Ann. § 63-19).
Provide clinical consultation, periodic chart reviews, and collaborative oversight for the Physician Assistant team.
Conduct a minimum of one on-site facility visit each month, with additional visits based on your availability.
Evaluate and manage your own panel of post-acute patients, focusing on musculoskeletal conditions, pain management, and rehabilitation.
Bill independently for professional services provided to your own patients.
Perform or oversee bedside ultrasound-guided musculoskeletal injections, if qualified.
Collaborate with facility staff and interdisciplinary care teams to optimize patient outcomes.
Qualifications
MD or DO with an active, unrestricted Tennessee medical license (or eligibility to obtain one)
Board Certified or Board Eligible in one of the following:
Experience caring for Skilled Nursing Facility (SNF), rehabilitation, or post-acute patients preferred
Comfortable managing conservative musculoskeletal and chronic pain conditions
Active DEA registration
Medicare enrollment (or eligibility)
Experience with ultrasound-guided musculoskeletal injections is preferred but not required
Compensation
Monthly collaboration stipend: $3000-$6,000, based on the size of the Physician Assistant team supported
Earn a generous percentage of collections for patients you personally evaluate and treat
Flexible 1099 independent contractor engagement
Malpractice contribution negotiable
Physical Requirements
Ability to travel between facilities within the assigned coverage area
Frequent standing, walking, and sitting during patient care and documentation
Ability to assist with patient positioning and lift up to 50 lbs when necessary
Compliance with facility PPE, infection control, and vaccination requirements
Ability to work safely in clinical environments with potential exposure to bloodborne pathogens
