Location: Nogales, AZ

Dependable Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice team in Nogales, Arizona on a part-time basis.

This is a meaningful opportunity for an LPN who is passionate about providing comfort-focused, patient-centered care to individuals and families during an important stage of life. You'll work alongside a supportive interdisciplinary hospice team while helping patients remain comfortable and cared for wherever they call home.

Responsibilities
  • Provide skilled nursing care to hospice patients under the direction of the RN and established plan of care

  • Monitor patients for changes in condition, symptoms, and comfort needs

  • Administer medications and treatments as ordered and within LPN scope of practice

  • Assist with pain and symptom management

  • Provide compassionate support and education to patients and their families

  • Communicate changes in patient condition to the RN Case Manager and hospice team

  • Collaborate with nurses, physicians, social workers, chaplains, aides, and other members of the interdisciplinary team

  • Maintain accurate, timely, and complete clinical documentation

  • Promote patient comfort, dignity, safety, and quality of life throughout hospice care

Requirements
  • Current and active LPN license in the State of Arizona

  • Hospice, home health, skilled nursing, or direct patient care experience preferred

  • Strong communication and interpersonal skills

  • Ability to work independently in a home-based care environment

  • Compassionate and patient-centered approach to care

  • Reliable transportation and a valid driver's license

  • Ability to travel locally throughout the Nogales service area

  • Bilingual English/Spanish is required.
