We provide compassionate, evidence-based psychotherapy and psychiatric care in the comfort of patients' homes, assisted living facilities, and independent living settings.

We're expanding our team and are looking for aLicensed Clinical Social Worker (LCSW)who shares our commitment to making mental health care accessible and deeply personal.

Position Overview

As an LCSW with Mindful Mobile Medicine, you will:

  • Provide in-person psychotherapy services to older adults and other individuals living in private homes, assisted living, and independent living environments.
  • Collaborate closely with our Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver integrated, patient-centered care.
  • Assess mental health needs, develop treatment plans, and provide individual therapy.
  • Maintain thorough and timely clinical documentation.

Qualifications

  • Required:Active LCSW license in Utah
  • Required:Ability and willingness to work in the community (travel to homes and facilities)
  • Strong clinical skills and a compassionate, client-centered approach
  • Experience with older adult populations or in-home/community-based care is a plus
  • Reliable transportation and valid driver's license
