We have an opening for a part-time/PRN Licensed Practical Nurse, LPN, or Registered Nurse, RN, with a 12 hour night position (6 PM to 6 AM, Tuesdays & Wednesdays Rotating) with that includes a great shift differential and skilled nursing incentive! Provide compassionate and effective care and treatments to our residents.
  • Support the Director of Nursing with assessments, admissions, and other duties as assigned.
  • Work with a team of professionals to ensure the residents comprehensive needs are met.
  • Review routines, calendar of events and resident needs
  • Ensure medications and supplies are ordered, in stock and counted
  • Ensure all charting and record keeping is accurate and complete
  • Promote the Mission, Vision and Values of Transforming Age, our Culture of Excellence, in all areas of responsibility.
  • Perform other duties as assigned.

What you need to succeed!
  • Be a graduate of an accredited LPN/RN program
  • Have an active LPN/RN license in the state, in good standing
  • Experience in Long-term Care
  • Have flexibility, able to work the occasional weekend/holiday as needed.
  • Provide leadership and supervision to nursing team members
  • Have knowledge of DHS Regulations
  • Possess superb interpersonal and communication skills and the ability to work well and collaborate with team members and external constituents
  • Have first aid/CPR certificate per state/local guidelines.

Physical Requirements:
  • On occasion, may need to push/pull/support up to 50lbs.
