Description
The Home Health Registered Nurse (RN) provides high-quality, patient centered nursing care to individuals in their homes. You will assist in delivering and managing care for patients with various medical conditions, including those recovering from surgery managing chronic illnesses, or other health related concerns. Your role is vital in ensuring patients receive compassionate, efficient, and effective care in a home environment.
Schedule: PRN, Weekdays, Weekends, On-Call, Flexible Availability
Primary Responsibilities and Duties:
  • Administer medications and treatments as prescribed by physicians and in accordance with home health care protocols.
  • Perform routine and specialized nursing tasks, including wound care, catheter insertion, and monitoring vital signs.
  • Assist in developing and implementing individualized care plans in collaboration with the patient, family, and healthcare team.
  • Monitor and document patient progress, response to treatments, and any changes in condition, reporting significant findings.
  • Provide education and support to patients and their families on disease management, medication use, and home care techniques.
  • Assist with patient mobility, transfers, and activities of daily living as needed, ensuring safety and comfort.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
  • Collaborate with LPNs, physicians, and other healthcare professionals to ensure coordinated and comprehensive care.
  • Respond to patient needs and changes in condition promptly, providing appropriate interventions and support.
  • Adhere to infection control and safety procedures to protect patients and staff.
  • Perform all other duties assigned.

Requirements
Job Qualifications:
  • Valid Registered Nurse (RN) license in the state in which work is performed.
  • 1- 2 years of nursing experience as an RN preferred, with a preference of experience in home health.
  • Strong clinical skills and the ability to perform a range of nursing procedures with precision.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.

  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.

  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
