Position Summary (General Purpose)

The primary purpose of the LVN position is to provide direct nursing care to residents and to oversee the day-to-day nursing activities performed by nursing assistants. This role ensures compliance with federal, state, and local regulations and supports the facility's commitment to maintaining the highest quality of care.

Essential Duties and ResponsibilitiesAdministrative & Leadership Functions
  • Direct day-to-day activities of nursing assistants in accordance with facility policies and regulations.

  • Ensure nursing staff compliance with established policies, procedures, and job expectations.

  • Participate in staffing coordination, shift planning, and resident care coordination.

  • Support Quality Assurance/Performance Improvement and participate in surveys as requested.

  • Assist with admissions, transfers, discharges, and incident/accident reporting as needed.

  • Maintain confidentiality and safeguard resident/facility information and access credentials.

  • Promptly report suspected unauthorized access attempts or confidentiality breaches.

Charting & Documentation
  • Complete required admission/transfer/discharge documentation accurately and timely.

  • Transcribe and process physician orders per facility procedure.

  • Maintain thorough nursing documentation reflecting care provided and resident response.

  • Communicate discrepancies or concerns regarding orders, documentation, or diet changes to the Nurse Supervisor.

Medication & Treatment Administration
  • Prepare and administer medications and treatments as ordered.

  • Follow resident identification protocols prior to medication/treatment administration.

  • Maintain accurate narcotic counts and report discrepancies promptly.

  • Ensure proper ordering, storage, and disposal of medications per policy.

Resident Care Responsibilities
  • Provide direct nursing care in accordance with care plans and physician orders.

  • Monitor resident condition and report changes to physicians and responsible parties as appropriate.

  • Assist in restorative/rehabilitative nursing programs and support resident self-help goals.

  • Ensure resident rooms and treatment areas are clean, safe, and prepared for admissions.

Safety & Infection Control
  • Ensure compliance with infection control practices and hand hygiene protocols.

  • Monitor and enforce safety procedures and appropriate use of PPE.

  • Report occupational exposures and incidents according to facility policy.

Supervisory Responsibilities

As an LVN, you are delegated the administrative authority, responsibility, and accountability necessary to carry out assigned duties and provide leadership to nursing assistants on shift.

QualificationsEducation and Experience
  • Graduate of an approved LVN program (or equivalent).

  • Current, active, and unencumbered LVN license in the State of California.

  • Skilled nursing or long-term care experience preferred.

Skills
  • Ability to read, write, speak, and understand English.

  • Strong clinical judgment and ability to work independently.

  • Ability to maintain professionalism with residents, families, staff, and visitors.

  • Knowledge of nursing practices, procedures, and SNF regulations.

  • Proficiency with documentation systems; PointClickCare (PCC) experience preferred.

Licenses/Certifications
  • Current LVN license (CA) in good standing

  • Current CPR certification

  • Must maintain continuing education and licensure requirements

Physical Demands & Work Environment

This role requires frequent movement throughout the facility and the ability to support resident care needs, including lifting, pushing/pulling, and assisting with transfers. The work environment includes resident rooms, nursing stations, and treatment areas. Exposure to infectious diseases and healthcare-related hazards may occur.
