Summary
We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will provide high-quality nursing care to our residents, ensuring their health and well-being while contributing to our mission of family-focused care.
This is a great opportunity for an LPN or RN looking to start or continue their career in a dedicated and compassionate environment. Our team is dedicated to providing the best quality care for the residents at our community.
Starting pay for this position, based on experience, is $28.50 an hour for LPNs and $40 an hour for RNs
The typical schedule for this opportunity is Three (3) 8-hour shifts a week. The standard schedule for this position includes an every other weekend rotation. Specific schedules available will be discussed during interview.
This position is available for 3rd Shift from 11pm - 7am.

Responsibilities
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient health and provide updates to the healthcare team.
  • Assist in the development and implementation of individualized care plans.
  • Perform assessments, including vital signs and medical history evaluations.
  • Educate patients and families on health management and wellness practices.
  • Coordinate with interdisciplinary teams to ensure comprehensive care delivery.
  • Document patient progress accurately in electronic health records.

Requirements
Requirements
  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Ohio.
  • Experience in Long Term Care, Behavioral Health or Acute Care preferred.
  • Strong clinical assessment skills and attention to detail.
  • Excellent communication and interpersonal skills for effective patient interaction.
  • Familiarity with electronic health record systems such as Point Click Care is a plus
