PHYSICAL THERAPIST HOME HEALTH – PART-TIME (South Hills, Pittsburgh, PA)

Due to strong community support, Bridges Therapy Associates is seeking a Part-Time Home Health Physical Therapist to join our team. The qualified candidate will provide effective, patient-centered services that will promote functional improvement and maximize patient potential in their home environment. The successful candidate will have a professional work ethic and a desire to make a positive difference in the lives of their patients.

RESPONSIBILITIES:

· Perform routine home physical therapy visits including evaluations, starts of care, daily visits, reevaluations, discharges

· Provide high quality patient care, evaluates functional needs and outcomes, and deliver effective and patient-centered treatments to achieve desired outcomes

· Designs and implements an appropriate plan of care for patients based on a thorough assessment of their functional needs in their home environment

· Conducts Physical Therapy practice within defined standards of care

· Communicates effectively with interdisciplinary team and support staff to achieve positive patient outcomes

· Work with and communicate effectively with PTA staff as appropriate

· Perform and document appropriate visits in timely manner

REQUIREMENTS:

· Proven work experience as a Physical Therapist preferred

· Valid professional PT license to practice Physical Therapy in Pennsylvania

· Valid driver’s license and auto insurance

· Ability to manage patients of varying demographics, backgrounds, and diagnoses

· Must be willing to travel to patient home

· Professional behavior

· Excellent communication and interpersonal skills
