Job description

Summary

The Home Infusion Pharmacist is responsible for overseeing the safe and effective management of medications for home Infusion patients. This role involves reviewing prescriptions, developing individualized medication plans, monitoring patient outcomes, and providing direct consultation to patients and caregivers. The pharmacist will work closely with interdisciplinary care teams to ensure seamless communication and coordination of care. Additionally, the Home Infusion Pharmacist ensures compliance with state and federal regulations and provides education and training to both patients and home health staff on medication safety and best practices.

Job Description:

Baptist Health is looking for a part-time Home Infusion Pharmacist to join our team at Baptist Health Lexington Home Care, 2100 Nicholasville Rd, Lexington, KY!

3 days per week from 9:00am - 5:30pm.

Alternating weekends and holidays on-call.

  • Oversee medication regimens for home health patients, ensuring safe and effective use.

  • Review and assess physician prescriptions for appropriateness and accuracy.

  • Develop and implement individualized medication plans based on patient needs.

  • Monitor patient outcomes, assess adverse drug reactions, and adjust medications accordingly.

  • Provide direct consultation with patients and caregivers on proper medication use, side effects, and potential drug interactions.

  • Work closely with home health nurses, physicians, and other healthcare providers to ensure a cohesive patient care plan.

  • Attend interdisciplinary team meetings to review patient progress and medication needs.

  • Serve as a resource for clinical staff regarding drug therapy and provide guidance on medication-related issues.

  • Facilitate communication between the pharmacy, home health team, and patient for timely medication delivery and adjustments.

  • Ensure compliance with state and federal pharmacy regulations related to home health services.

  • Maintain accurate and thorough documentation of medication management in patient records.

  • Conduct regular audits of medication records and processes to maintain regulatory standards.

  • Provide training and education to patients, caregivers, and home health staff on medication safety and best practices.

  • Educate home health clinicians on new medications, dosage adjustments, and treatment protocols.

  • Assist in developing educational materials and resources for patients and clinical staff on medication-related topics.

Minimum Requirements

  • Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.

  • Minimum of 1 year of experience required. Home health, hospice or similar care setting experience preferred.

  • Experience with chronic disease management, polypharmacy, and geriatric patient care is preferred.

  • Active, unrestricted Kentucky Board of Pharmacy license.
