Occupational Therapist Responsibilities

As a Home Health Occupational Therapist, you will:

  • Complete in-home Occupational Therapy evaluations and reassessments
  • Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs)
  • Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization
  • Recommend adaptive equipment, home modifications, and fall prevention strategies
  • Educate patients and caregivers on safety, mobility, and functional independence
  • Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team
  • Document patient progress accurately using EMR while maintaining Medicare and agency compliance
Qualifications
  • Current Arizona Occupational Therapist (OT) license
  • Graduate of an ACOTE-accredited Occupational Therapy program
  • Minimum one year of Occupational Therapy experience (Home Health preferred)
  • Strong communication, organization, and time management skills
  • Experience with EMR documentation
  • Valid driver's license, reliable transportation, and current auto insurance
