Job Summary
The Registered Nurse provides nursing care in accordance with the patient’s plan of care. Responsibilities include comprehensive health and psychosocial evaluations, monitoring conditions, promoting health and prevention, coordinating services, providing teaching and training, and delivering direct nursing care.
Key Responsibilities
  • Conduct comprehensive health and psychosocial evaluations to coordinate total patient care.
  • Monitor patients’ conditions and promote sound preventive practices.
  • Evaluate the effectiveness of nursing services provided to patients and families on an ongoing basis.
  • Perform patient admissions, transfers, re-certifications, resumptions of care, and discharges in compliance with care plans.
  • Consult with attending physicians to adjust patient care plans in collaboration with supervisors.
  • Coordinate patient services across the healthcare team.
  • Submit clinical notes, progress notes, and other clinical documentation regularly, as required.
  • Maintain daily logs of patient care visits and submit them accordingly.
  • Participate in case conferences and discuss patient care concerns with supervisors and other healthcare team members.
  • Coordinate with other healthcare professionals, including Home Health Aides, Physical Therapists, Speech Therapists, Occupational Therapists, and Medical Social Workers, to optimize patient outcomes.
  • Perform other duties as assigned.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Active Registered Nurse license in Pennsylvania.
  • At least one year of experience in a community/home health agency or home care setting preferred.
  • Knowledge of Medicare and Medicaid guidelines.
  • Working knowledge of home healthcare principles, professional nursing techniques, and required documentation.
  • Strong organizational and time-management skills.
  • Effective communication skills to contribute to the quality of care through collaboration with managers and staff.
  • Current CPR certification.
