You will deliver care aligned to the established plan of care, collaborate with the interdisciplinary team, and maintain accurate, timely documentation while safeguarding PHI.Key responsibilities include completing in-home skilled visits; assessing patient status; delivering ordered treatments; providing patient and caregiver education tied to diagnosis and care plan; coordinating services and communicating needs for other disciplines (HHA, OT, PT, MSW, ST, and paraprofessionals); and partnering with the Clinical Manager for ongoing performance and development.Required qualifications/skills:- Active RN license- Current CPR certification- Current TB test- Fingerprint Clearance Card- Valid driver’s license, reliable personal vehicle, and active auto insurance- Strong clinical judgment, communication, time management, and compassionate customer serviceIf you value one-on-one care in the home setting, submit your resume today.Typical WorkdayYour weekday schedule (Monday–Friday) starts by reviewing your visit plan and confirming timing with patients and caregivers.
