The Home Health Registered Nurse (RN) plays a critical role in delivering skilled nursing care and comprehensive case management services to patients in their homes. In this role, the RN functions as both a clinician and a case manager-conducting holistic assessments, developing and managing individualized plans of care, coordinating services across the interdisciplinary team, and monitoring patient progress throughout the episode of care. The Home Health RN serves as the primary point of clinical oversight, ensuring care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals, while supporting successful outcomes and transitions across the continuum of care.

Pay Range: $34.00 - $49.18/hour

Employment Type: Part Time, 48 hours every 2 weeks

Schedule: Day shift, Typically 8 hour days.

On-call: Approximately once every 3 weeks during the week and every 4th weekend.

Requirements:Graduate of accredited Registered Nursing Program required, Current Michigan RN License (or completed by end of orientation) required, Current BLS for the Healthcare Provider (or completed within 60 days of hire) required

1. Skilled Patient Care & Clinical Assessment

  • Conducts comprehensive admission, recertification, and discharge assessments in the home setting

  • Develops, implements, and updates individualized plans of care in collaboration with the physician and care team

  • Provides skilled nursing services including medicationeducation, wound care, symptom management, IV therapy, and chronic disease management

  • Monitors patient condition, identifies changes, and initiates appropriate clinical interventions

2. Patient & Family Education and Engagement

  • Educates patients and caregivers on diagnoses, medications, treatments, and selfmanagement strategies

  • Communicates clearly and professionally, adapting education to patient needs, learning styles, and health literacy

  • Promotes patient dignity, privacy, and confidentiality during all interactions

  • Encourages patient independence and safe care practices in the home

3. Interdisciplinary Collaboration & Care Coordination

  • Collaborateswith physicians, therapists, social workers, and other care team members

  • Participates in case conferences and contributes to coordinated care planning

  • Communicates patient status and concerns effectively using verbal, written, and electronic methods

  • Demonstrates accountability, professionalism, and problemsolving in daily practice

4. Documentation, Quality & Safety

  • Completes accurate, timely documentation using electronic health records and ambient AIsupported documentation tools

  • Ensures compliance with agency policies and state and federal regulatory requirements

  • Follows patient safety, infection control, and environmental safety standards

  • Participates in quality improvement activities and maintains required competencies
