JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)This position will be hired as PRN. Part-Time is also available with a 2-hour travel radius to "Secondary" locations listed below.Full-Time may be available in the future but not at this time and is not guaranteed.This is a regional leadership role within our agency, requiring travel as needed throughout Southwest Ohio where clients and caregivers reside.This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate). For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
