LifeCare is hiring immediately for a Part time Home Health Nurse.

LifeCare is looking for experienced, committed, passionate registered nurses (RN) to join our growing field home health team to serve the Chicago area. This staff nurse is a licensed professional committed to delivering quality home care services to our homebound patients. These services are provided with physician orders and the nurse will report to a clinical manager for supervision and support.

Start of Care $75

QUALIFICATIONS

  • Graduate of an approved of professional nursing program. It is preferable that the staff nurse has completed a baccalaureate degree program approved by the National League for Nursing for public health nursing preparation
  • Current Illinois registered nurse license in good standing.
  • Minimum of one (1) year of home health experience, including OASIS training required
  • Current CPR certification
  • Detail-oriented, well organized flexible professional who delivers patient-centered care.


JOB RESPONSIBILITIES

  • Performs assessment and start of care within 24-48 hours of referral,
  • Develops a plan of care to achieve clinical outcomes through short and long term goals that are realistic and appropriate.
  • Provide skilled nursing care as outlined in the nursing care plan to include the following:


Nursing services, treatments and preventative procedures requiring substantial specialized skill and ordered by the physician Initiating preventative and rehabilitative nursing procedures as appropriate for the clientas care and safety

  • Perform duties of the case manager to ensure coordinated plan of care among all disciplines, including therapy services, home health aides.is offered to patient.
  • Evaluate the patient and, family situation to determine if additional community resources are necessary to ensure safe discharge from skilled services
  • Submit all documentation within 24 hours of patient visit, allowing for exception for completion dates within OASIS.
  • Actively participants in case conference, team meetings, educational opportunities and quality improvement projects to remain engaged in appropriate interdisciplinary activities.


Meets regularly with clinical manager to discuss patient status, non-emergent concerns.
