The Care Navigation Nurse LPN supports referral intake, care coordination, and initial clinical triage to ensure timely, accurate, and efficient access to hospice services. The role emphasizes one-call resolution whenever possible and seamless progression of referrals through dispatch. 

WHAT YOU WILL DO: 

• Retrieve, monitor, and process referrals for hospice services and Western Reserve Navigator potential patients from multiple sources, including: o CarePort o Electronic fax inbox o Phone referrals o Other designated referral sources • Learn and utilize the dispatch process to schedule appointments for prospective patients. • Follow up on referrals that do not progress to dispatch in a timely manner. • Perform direct admits to inpatient care centers as needed. • Take clinical calls from patients and caregivers; attempt resolution and escalate appropriately. • Notify primary care teams of any clinical issues addressed. • Participate in quality improvement and process improvement initiatives. • Perform other duties as assigned.

 

SUCCESS CAPABILITIES:

• Licensed Practical Nurse (LPN) in the state of Ohio • Hospice experience strongly preferred • Excellent verbal and written communication skills • Ability to communicate calmly and compassionately with patients and families in distress • Strong teamwork and collaboration skills • Comfortable using multiple technologies, including: o Electronic Medical Records (EMR) o Phone systems o Referral and intake platforms

PHYSICAL REQUIREMENTS:

This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemical and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
