Your Role
  • Manage Your Caseload: Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
  • Skilled Care: Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
  • Coordinate: Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
  • Educate: Empower patients and families to manage their conditions safely at home.
  • Document: Maintain accurate, timely clinical notes on company-issued iPads.
Requirements
  • LICENSE: Current, active California RN License is REQUIRED to apply. Applications without a valid CA license number will not be reviewed.
  • Experience: Minimum 1 year of professional nursing experience required. Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.
  • Travel: Valid California Driver's License and reliable transportation are mandatory.
