Job Overview:
We are currently seeking compassionate and skilled Home Health Licensed Practical Nurses – LPN to provide personalized, one-on-one patient care throughout Buchanan, Russell, and Tazewell counties. This position is ideal for an LPN who enjoys working independently while advocating for patients and helping them achieve the best possible outcomes within the comfort of their home.

As a Home Health Licensed Practical Nurse – LPN, you will provide home-based care for patients recovering from illness, injury, surgery, or chronic conditions while working closely with RNs, physicians, and interdisciplinary care team members.


Responsibilities of Home Health Licensed Practical Nurse – LPN:

  • Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions under the supervision of an RN
  • Monitor patient condition, obtain vital signs, and perform routine nursing assessments
  • Document patient progress and report changes in condition to RN supervisor
  • Assist with implementing individualized patient care plans
  • Educate patients and families on medications, treatment plans, self-care, and home safety
  • Collaborate with physicians, RNs, CNAs, aides, and interdisciplinary team members to coordinate patient care
  • Assist with discharge planning and ensure continuity of care
  • Maintain accurate and timely documentation

Qualifications of Home Health Licensed Practical Nurse – LPN:

  • Graduate of an accredited practical nursing program
  • Active LPN license in the state of Virginia
  • Minimum of 1 year of LPN experience (home health preferred) or completion of office preceptor program
  • CPR certification and TB skin test required
  • Knowledge of state and federal home health regulations
