Home Health Registered Nurse - part-time needed!

Pay: This is a paid per visit position with routine visit rates starting at $55 per visit.Β

TheΒ RN's overall responsibilityΒ is to provide direct patient care and case management to an assigned caseload in the home setting in accordance with physician’s orders. This includes admission of patients for service and determining and developing a plan of care.

Duties Include:

Β· Obtains medical history, particularly as it relates to the present condition, from the patient and/or family member.

Β· Conducts a brief physical examination of the patient, including vital signs, ambulatory status, mental status, bowel and urinary regularity, appetite and type of diet.

Β· Interprets nursing services of the agency to patients and families as part of planning of care.

Β· Provides skilled care and maintains confidentiality of client’s situation, records and services provided according to HIPAA regulations.

Β· Evaluates the effectiveness of his/her nursing service to the individual and family.

Β· Submits all patient notes and/or visit records on a timely basis.

Β· Develops, implements, participates in the nursing care plans and coordinates total patient care.

Β· Evaluates the patient’s condition and home situation, determine if the services of a LPN, Home Health Aide, Personal Care Attendant will be required and the frequency of this situation.

Β· Obtains orders for paramedical services and submits referral to the Director of Clinical Services, i.e., physical therapy, medical social services, etc.

Β· Coordinates and supervises the care given by the LPN, Home Health Aide and/or Personal Care Attendant assigned to provide care to the patient.

Qualifications Include:

Β· Licensed RN

Β·Β OASIS and Wellsky (Kinser) experience preferred, we provide training.

Β· Med-surge and wound care experience preferred.

Β· Physical within the last year

Β· Current TB results (within last 12 months) and CPR Certification.

Β· Valid driver’s license and proof of liability insurance on automobile, driving history report.

Β· Ability to attend orientation at our Willowbrook office for 1-3 days
