Haven's Gateway Personal Care Facility Inc. for over 18 years is seeking a Direct Service Worker to join our team! You will assist in the daily care of elderly or disabled individuals. We offer part time flexible hours ALL shifts unlimited hours! If you are compassionate about people, please apply today.

Responsibilities:

  • Assist clients with daily living activities on a daily basis based on the clients comprehensive Plan OF Care
  • Communicate ongoing care results and updates to supervisors and written daily notes
  • Collaborate with clients for the best care based on their CPOC
  • Maintain a healthy and comfortable living environment
  • Ensuring that the client makes all assigned appointments

Qualifications:

  • Previous experience in personal care or other related fields 6 months
  • Compassionate and caring demeanor
  • Ability to build rapport with clients
  • Excellent written and verbal communication skills
  • Background Check
  • Reliable transportation
  • Valid Drivers License and Auto Insurance
  • Social Security Card
  • 18 or older
