Evening Shift LPN

Job Summary:

We are seeking a dedicated and compassionate Evening Shift LPN to join our team at Norwalk Care Center, a leading provider of healthcare services in Norwalk, Connecticut. As an Evening Shift LPN, you will play a vital role in delivering high-quality patient care and support to our residents. As an Evening Shift LPN, you will be responsible for ensuring the health and well-being of our residents within a dynamic and fast-paced environment.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and other treatments as needed

  • Develop and implement individualized care plans for residents, in collaboration with the healthcare team

  • Conduct regular assessments and monitoring of residents' condition, reporting any changes or concerns to the Charge Nurse or Physician

  • Maintain accurate and up-to-date records of patient care and treatment

  • Participate in inter-disciplinary team meetings and contribute to the development of care plans

  • Provide emotional and social support to residents and their families

  • Collaborate with other healthcare professionals to ensure seamless care delivery
